Your maintenance care for the year comes with it: four perio maintenance visits, an exam at each visit, annual bitewing X-rays, fluoride at every visit, and one emergency exam.

Perio Maintenance Schedule (D4910)
4 Perio Maintenance Visits Per Year, Every 3 Months
What is D4910? Perio maintenance (D4910) is a therapeutic procedure for patients who have completed active periodontal therapy. Unlike a standard cleaning, it includes debridement of the sulcular epithelium and removal of plaque, calculus, and bacterial toxins from above and below the gumline, critical for preventing disease recurrence.

It is not regulated as insurance under California law, provides no coverage for services outside D Street Dental Group, and does not qualify as minimum essential coverage.
Common Questions
Is the membership the same as dental insurance?
No. It's a plan we run in-house, not insurance. You pay us directly, so there are no claim forms, no deductibles, and no waiting periods. It covers your care here at D Street Dental Group only. If that sounds simpler than insurance, that's the point.
If I need a filling or a crown, do I still pay for it?
Yes, but you get 20% off. The membership covers your four perio maintenance visits, your exams, and X-rays. Anything beyond that, like fillings, crowns, implants, or whitening, comes off our standard fee by 20%. Clear aligners and cosmetic-only work like veneers are the two things the discount doesn't touch.
What if I already have dental insurance?
Then this probably isn't the plan for you, and that's fine. We built it for patients without dental benefits. We're out of network with the PPO plans, so if you're covered we'll bill your plan directly and you use your out-of-network benefits instead. You can't run both at the same time.
How soon can I use it?
Right away. There's no waiting period. Your benefits start the day you enroll and run for a full 12 months. We'll get your first maintenance visit on the calendar.
What happens to the visits I don't use?
They don't roll over. The plan resets each year when it renews, and with periodontal maintenance the timing matters, so staying on the every-three-months schedule is the whole point. We'll help you keep all four visits booked.
How do I sign up?
Call, text, or email us and we'll have you enrolled in a few minutes. Online sign-up is coming soon. Either way, you don't need insurance, a referral, or any paperwork ahead of time.
Why four cleanings a year instead of the standard two?
Because you've already had gum disease treated, and four visits a year are what keep it from coming back. Every three months we clear the bacteria from under the gumline before it builds back up. That buildup is what caused the gum disease in the first place, so a standard twice-a-year cleaning isn't enough to hold it off. Staying on the three-month schedule protects the work you've already done.
Is whitening part of the 20%?
Yes. Professional whitening is 20% off as a member. Veneers, other cosmetic-only work, and clear aligners are the exceptions.
